As a single document, “Registration” is comprised of SEVEN sections: PERMISSION TO PARTICIPATE, BUDDY TEAM PLEDGE, LOANER TOOLS AND KIT PICKUP/RETURN, STUDENT INFORMATION, PARENT/GUARDIAN INFORMATION, PUBLICITY RELEASE, and SIGNATURE.

1. PERMISSION TO PARTICIPATE

By reading and completing all SIX sections of this “Registration” and subsequently electronically signing and submitting same to **Toolbox, Inc. for acceptance, I hereby give you permission for my child/5th-grade student to participate in the following club:


Young Apprentice Construction Club


This club does not meet in person or virtually online, but rather follows a thirteen lesson format collectively referred to as the 'Young Apprentice Experience'. This course is a self-paced learning 'Experience' which teaches basic construction tool skills while building confidence, pride, and self-esteem.

2. BUDDY TEAM PLEDGE

I understand that my child is signing up for a Fulton County Schools sponsored club that developed the Young Apprentice Experience online training course as described herein. I also understand that this program uses a parent/student “BUDDY TEAM” model which requires my dedicated participation. I, therefore, promise to be present to observe each lesson and watch the lesson videos in their entirety in order to participate in full support of my child.


In this role, I hereby promise to inspect all of my child’s homework and quizzes, the first of which is to be completed at the end of the 'Welcome' video. I further promise to tutor her/him whenever necessary, so that she/he might learn the intended material presented in the various short-term videos which make up much of the Young Apprentice Experience course curriculum.


Quizzes and Homework:

- Lessons 1-13 have quizzes, each of which must be completed in order to advance to the next lesson.

- Tool and Material Identification Lab: introducing students to the tools and materials that are involved in making the ‘PERFECT TOOLBOX’ project possible.

- One Inch Dragon Lab: a practical exercise helping students visualize fractions when reading a tape measure.

- Tape Measure Lab-2: giving students 10 opportunities to challenge their new skill.

- Speed Square Lab: a practical exercise combining the Tape Measure and Speed Square.

- PERFECT TOOLBOX-1: teaching students and BUDDIES how to properly use a Bench Hook when building their PERFECT TOOLBOX.

- Last lesson (Job well done) - No homework.


I promise to watch all videos with my child before my child attempts any construction of their ‘PERFECT TOOLBOX’. Also, I promise to watch all of the videos in order and follow the instructions outlined in each until the ‘PERFECT TOOLBOX’ project is completed.


As my child’s BUDDY, I will never permit my child to work on this project without my full presence, full attention, and full assistance. However, I understand that I am not to use any of the tools. Those are for the exclusive use of my child, so that she/he is the only one to build the ‘PERFECT TOOLBOX’.


All videos from completed lessons may be rewatched by returning to the lesson of interest. However, once all quizzes are taken and successfully passed such that the course becomes completed, there is no opportunity to revisit any video content.

3. LOANER TOOLS AND BUILD KIT PICKUP/RETURN

I PROMISE to return the tools and materials borrowed from Toolbox, Inc., so that my child could participate in the 'Young Apprentice Experience'. I will return all borrowed tools and materials as listed on the 'Loaner Tool and Material Return' form which was provided to me in the black (or orange) student binder that was part of the 'PERFECT TOOLBOX Build Kit'.


The Build Kit pickup window is 8 AM - 11 AM.

If you pick up your Build Kit on Mar19, it must be returned on Apr16 between 8AM - 11 AM at the location you select from the drop-down box below.

If you pick up your Build Kit on Mar26, it must be returned on Apr23 between 8AM - 11 AM at the location you select from the drop-down box below.

4. STUDENT INFORMATION

5. PARENT/GUARDIAN INFORMATION

Throughout your child’s participation in the Young Apprentice Construction Club, many companies and organizations in our community have sent volunteers, donated goods and/or services, and have otherwise shown their support for this initiative. We would like to document the success of the program by taking pictures or videos of action during the club which may include your child. With your consent, these images of the activities your child is participating in may be publicized through local or national media to promote the initiative of having this type of program at the elementary school level.

6. SIGNATURE

By affixing my signature electronically in the space below, it is to affirm the truth and accuracy of all representations made by me in this “Registration”, which furthermore affirms my understanding of the BUDDY PLEDGE and inherent promise to fulfill my duties as outlined therein.
